Medicare does not pay for an in-home caregiver when custodial care services such as housekeeping and personal care are all you require. Medicare might pay for some short-term custodial care if it is medically necessary and your doctor approves that you are homebound.
Medicare does pay for home health care services such as physical therapy, skilled nursing care, occupational therapy, speech therapy, and social services if you are homebound after surgery, an ailment, or an injury. Your doctor will require certifying that the services are medically required, and your home health agency needs to be Medicare-certified.
